This is Mishek and (some of) his family. Mishek is one of two watchmen that guard the clinic on a 24hour basis.

They work alternative 12 hour shifts, rotating the night-shift on a weekly basis. For working approx 360 hours a month, they are paid £12 (and with this guaranteed income they feel like they've won the lottery.)
